Services

Payment for services is due at the time of service. No payment plans or deferred billing is available. Payment can be made in cash (exact amount only), check, or credit/debit card. Insurance coverage will be out of network for all providers. I am not paneled with any insurance companies. The rate of reimbursement depends on your specific insurance plan. I would encourage you to call your provider to understand if you have a separate  deductible for out of network services (if separate from in network) and what the coverage rate for CPT 90834 Individual Counseling will be.

I conduct several types of evaluations so it is helpful if you are specific in your request for services as to which service you are looking for. 

General Mental Health & Substance Use Evaluations are for therapy and do not require any documentation.

ASAP Evaluations require minimal documentation back to your case manager or probation counselor. 

Pre-court Evaluations are often requested by your attorney or pre-trial probation officer. They require a thorough review of records available, a general mental health and substance use evaluation, urine drug testing, questionnaires, and a summary document provided to the requested persons. 

Individual therapy is often weekly 50 minute sessions that may taper to less frequent sessions as progress is achieved. Therapy is mostly still over Zoom but I have one day a week I am in my office. 

Group therapy is weekly and I offer a mens and womens substance use group each of which are a minimum of 16 weeks in duration. Currently they are offered over Zoom. 

Phone calls longer than 15 minutes are billed at the hourly rate of $210/hr in quarter hour increments. 

I have a 24 hour cancellation policy for all services. The client portal will send you a reminder for your session 24 hours in advance of your appointment. If you need to cancel or reschedule your appointment you must do so with at least 24 hours notice so I have enough time to offer the time to another client. If you cancel with less than 24 hours notice you will be billed for the entire cost of the appointment. Cancellations must be made by text or email to your clinician.
